Clinical Criteria

When to use
  • Rapid bedside assessment of patients with suspected infection
  • qSOFA ≥2 → assess full SOFA, take blood cultures, check lactate
  • Not a diagnostic tool — a screening prompt for sepsis pathway
  • Use in ward / ED / pre-hospital settings (not ICU — use SOFA)
Formula
qSOFA = RR ≥22 + SBP ≤100 + GCS <15
Range: 0–3
≥2 = high risk for organ dysfunction
