Interpreting GRACE Score
In-hospital mortality:
Low: GRACE ≤108 (<1%)
Moderate: GRACE 109–140 (1–3%)
High: GRACE >140 (>3%)
6-month post-discharge mortality:
Low: GRACE ≤88 (<3%)
Moderate: GRACE 89–118 (3–8%)
High: GRACE >118 (>8%)
When to use
- All ACS presentations — STEMI, NSTEMI, unstable angina
- Guides timing of angiography (high risk → early invasive strategy)
- NICE NG185: GRACE >3% at 6 months → offer angiography within 72 hours
- Use alongside TIMI and clinical judgment
