HEART Score Chest Pain
History, ECG, Age, Risk factors, Troponin — predicts 6-week major adverse cardiac event (MACE) risk in chest pain.
—
Complete all 5 components
Score each of the 5 components to calculate 6-week MACE risk.
H — History
E — ECG
A — Age
R — Risk Factors
T — Troponin
6-Week MACE Risk
Score 0–3: Low risk (~2%) → Consider safe early discharge
Score 4–6: Moderate risk (~12%) → Observe, serial troponins
Score 7–10: High risk (~65%) → Early invasive strategy
When to use
- Undifferentiated chest pain in ED — combined with hs-troponin pathway
- HEART ≤3: validated for safe early discharge in multiple studies
- HEART ≥7: cardiology review and early angiography
- Does not replace clinical judgment or troponin kinetics
